Exposure and Response Prevention (E/RP)

Exposure and response prevention (also known as exposure and ritual prevention; ERP or EX/RP) is a variant of exposure therapy that is recommended by the American Academy of Child and Adolescent Psychiatry (AACAP), the American Psychiatric Association (APA), and the Mayo Clinic as first-line treatment of obsessive compulsive disorder (OCD) citing that it has the richest empirical support for both youth and adolescent outcomes.

ERP is the gold standard of treatment for OCD and OCD related disorders, and is a subset of CBT. Individuals with OCD and OCD related disorders often experience both intrusive and repetitive thoughts (obsessions) and the inability to avoid certain behaviors that are used to reduce or neutralize the intrusive thoughts (compulsions). ERP works in that it exposes an individual to the thoughts, images, and objects that make a person anxious (exposure), and then teaches the client how to avoid giving into the compulsive behaviors by sitting with and managing their anxiety (response prevention). Most individuals reading this are probably thinking to themselves ‘This sounds like terrible torture!’ However, each clinician works with each client in partnership and comes up with a hierarchy of obsessions and compulsions, guiding the treatment to allow the client to not become overwhelmed. Therefore, ERP can be successfully used with minimal discomfort and anxiety when the client feels the freedom to express where they would like to start and what they feel capable of doing.
