Exposure and Response Prevention (ERP, EX/RP)
Exposure and Response Prevention (ERP) is the gold-standard, evidence-based treatment for Obsessive-Compulsive Disorder (OCD) and related conditions, and is a specialized form of Cognitive Behavioral Therapy (CBT). Individuals with OCD often experience intrusive, unwanted thoughts, images, or urges (obsessions), along with repetitive behaviors or mental rituals (compulsions) performed to reduce distress or prevent feared outcomes.
ERP works by gradually and systematically helping individuals face the thoughts, images, objects, and situations that trigger anxiety (exposure), while learning to resist engaging in compulsions (response prevention). In doing so, individuals build tolerance to anxiety and uncertainty, and over time, the intensity and frequency of obsessive-compulsive symptoms decrease.

At first glance, many people think, “This sounds like torture.” In reality, ERP is highly collaborative, structured, and individualized. Clinicians partner closely with each client to develop a personalized hierarchy of obsessions and compulsions, starting with more manageable steps and progressing at a pace that feels achievable. Clients are never forced into exposures they are not ready for and are encouraged to have an active voice in where treatment begins and how it unfolds.
When implemented thoughtfully, ERP does not aim to overwhelm—it aims to empower. With the right support, individuals learn that they can experience anxiety without needing to escape or neutralize it, leading to increased confidence, flexibility, and meaningful, lasting symptom relief.
Intensive Treatment Approach
At Mid Island Psychology Associates, we recognize that symptoms and treatment needs vary widely, and some individuals benefit from a more concentrated level of care to achieve meaningful symptom reduction. For these cases, we offer intensive treatment programs designed to accelerate progress and improve outcomes.
Our intensive approach includes extended and higher-frequency sessions, often multiple hours per day and several days per week, depending on clinical need. Treatment may incorporate in-vivo (real-world) exposure, allowing clients to directly confront and work through symptoms in meaningful, everyday settings.
In addition, we utilize innovative tools such as virtual reality (VR) to enhance exposure-based treatment. VR allows for controlled, immersive experiences that can simulate real-life scenarios, making it an effective option for targeting anxiety, OCD, and functional neurological symptoms in a structured and supportive environment.
When clinically indicated, we also offer home-based sessions to address symptoms in the environments where they most naturally occur, further promoting generalization and long-term success.
